Non-urgent advice: Getting a sick (fit) note

A sick note is issued by certain healthcare professionals. It provides evidence of the advice patients have been given about their fitness to work.

A sick note explains details of your condition. This willl help you and your employer think about steps to help you return to work – this may be before your sick note expires.

Book an appointment with a GP to request a sick note for a new absence lasting over 7 days

To book an appointment to request a sick note please please phone the surgery from 08:30am on 01968 675 576 Monday - Friday.

Fit certificates can be back-dated so it is not necessary to make an emergency appointment to renew your certificate. If you attend an emergency appointment for the purpose of getting a medical certificate you will be asked to rebook. 

Request an extension to an existing sick note already issued by your GP

Please fill in this form to request an extension to your Sick Note.  Your sick note will be ready to collect from the surgery a minimum of 48 hours later.

Non-urgent advice: When do I need a sick note?

7 days off sick or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification.

More than 7 days off sick

If you’re off work sick for more than 7 days, your employer will usually ask for a sick note.
