Forms for you to submit online and securely to the practice

You can book an appointment with a GP for a new sick note or request an extension online.

Please telephone reception any afternoon, after 2pm, to check your test results.

If you live in Penicuik (and some surrounding areas) please pop in to the surgery to collect registration paperwork.
